By submitting this form you agree that the independent dealer named above is authorized by me to perform the needed described services and repairs, including replacement of necessary parts and to operate the vehicle for inspection, testing and/or delivery purposes. For vehicles which come in for diagnosis, a checking fee of $70 will be charged for the diagnosis, however, if the vehicle owner agrees to get the repair work done at S&A Service center, the fee may be waived. The estimated cost is acceptable to the undersigned and it is understood that the final invoiced price may exceed the estimate.
